A post implementation review that is not a formality
The review most projects run asks whether the project delivered. The useful one asks whether anything changed.
The standard post implementation review happens four weeks after go live, when the team is still being careful and the project manager is still in the room. It measures compliance with a launch, not adoption of a practice.
Run it at three months and again at six, with the same questions both times, and it starts telling you something. The interesting result is never the first number. It is the direction between the first and the second.
Ask about the practice, not the product. 'Is a change record created before the change?' is answerable and comparable. 'Are you satisfied with the new system?' is neither.
